说明：
1.     外国人就业证有效期满前30天内，用人单位填写此表（一份），办理延期手续。
2.     表中“审批机关”和“批准有效期限至”两栏由审批机关填写，用人单位不要填写。
3.     表中“代表机构有效期限至”一栏由代表机构填写。
4.     本表由用人单位用钢笔中文填写（网上办事除外）。
5.     外文名按护照填写。
6. 本表不能复印。
Further explanations:
1. The employer of the certificate holder shall fill this form and apply for extension of Alien Employment Permit in 30 days' advance before the expiring date.
2. The grids of "Approved by" and "Approved Validity of Extension" can only be filled by the approval Organization.
3. Only those representative offices need to fill the grid of "Validity of Representative Certificate".
4. This form should be filled in Chinese with fountain pen (Except for Application through Internet).
5. English name must be filled according to the passport.
6. This form could not be photocopied.
